Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist to provide outpatient rehab primarily in-home for older adults in Vernon and surrounding CT towns. The role covers a flexible schedule across home and facility settings, with opportunities to work across multiple sites.

The SLP will evaluate and treat communication and swallowing disorders, collaborate with the rehab team, and document care plans with patient and caregiver involvement. License and driver's license required.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient speech-language pathology services in home and facility settings.
• Supervise CFYs, students and support personnel as required by licensure standards.
• Adhere to department and patient treatment schedules with timely documentation and meetings.
• Collaborate with rehabilitation team and physicians to select appropriate therapeutic regimens.
• Adapt plan of care to patient needs and ensure safety and home environment readiness.
• Coordinate care in home and community with interdisciplinary team; perform other related duties.
