Qualifications

• Must hold a valid Speech-Language Pathologist license in the state of practice.
• Must have a valid driver’s license or reliable transportation.
• Experience in home-based or outpatient rehab is preferred.

Responsibilities

• Provide outpatient rehabilitation in-home or in facilities for older adults.
• Develop and implement speech-language pathology plans of care.
• Supervise CFYs, students and support staff per licensure.
• Maintain timely documentation and ensure schedule adherence.
• Collaborate with multidisciplinary rehab team to optimize outcomes.
