Job summary

TalentBurst, Inc is seeking a Speech Language Pathologist to provide outpatient rehabilitation services in the older adult population, primarily in-home. The role involves evaluating and treating patients in their homes or facilities, coordinating with the rehabilitation team, and ensuring high-quality patient care in a flexible setting.

Responsibilities include supervision of CFYs/students, adherence to treatment schedules, and developing individualized plans of care to restore communication

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice.
• Must possess or be eligible for ASHA CCC or CF eligibility.
• Must have a valid driver's license or reliable transportation to/from patients' residences.

Responsibilities

• Supervise CFYs, students and support personnel per state licensure requirements.
• Adhere to department and patient treatment schedules; complete clinical and billing documentation.
• Attend and contribute to patient care and staff meetings.
• Make referrals to other disciplines to address patient needs.
• Collaborate with rehab team and physician to select appropriate therapy.
• Adapt plan of care to patient needs; ensure safe home environment.
• Coordinate care in home/community with interdisciplinary team.
