Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home therapy, evaluate and treat patients, and support rehabilitation across home and facility settings.

The role requires ASHA Certification, a minimum of six months of experience, a current license, CPR, and reliable transportation for home visits. This is a full-time position with a competitive pay range and benefits.

Qualifications

• Certified as a Speech-Language Pathologist with ASHA CCC or ASHA credentials.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• CPR certification current and valid.
• A valid driver's license with reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in patients' homes or facilities.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapy.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Prepare and submit evaluations, visits, care plans, and progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and share information only with authorized persons.
