CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes, guiding rehabilitation and functional improvement.

You will assess speech, voice, language, swallowing disorders, develop plans of care, coordinate care with the care team, and document progress according to policy. A current ASHA CCC-SLP certification, license, CPR, and valid driver's license with reliable transportation are required.

Competenze

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Mansioni

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Maintain and submit documentation; prepare and submit reports as required by policy.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and professional conduct for protected information.
