Job summary

CenterWell Home Health is hiring a Home Health Speech Therapist to deliver SLP services in patients’ homes or facilities. You will participate in Plan of Treatment and Care, assess speech, voice, language and swallowing disorders, and maintain thorough documentation.

The role requires ASHA CCC eligibility or certification, six months of SLP experience, an active license and CPR, plus a valid driver’s license and reliable transportation. Join a team focused on rehabilitation and independence.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum of six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license; CPR certification.
• A valid driver’s license and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction in use of alternative communication systems when appropriate.
• Maintain documentation and submit progress reports as required.
• Participate in care coordination and discharge planning.
