Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Speech-Language Pathologist to provide in-home and community-based speech, language, and swallowing services to adults with developmental disabilities under NJ DDD guidelines.

You will perform assessments, develop individualized treatment plans, document progress, collaborate with families and support coordinators, train caregivers, and ensure compliance with DDD policies and HIPAA.

Qualifications

• Active New Jersey licensure as a Speech-Language Pathologist.
• ASHA CCC-SLP certification required; CFs considered with supervision.

Responsibilities

• Conduct comprehensive evaluations.
• Develop individualized treatment plans aligned with ISP goals.
• Deliver in-home and community-based therapy sessions.
• Document services per DDD standards and timelines.
• Collaborate with Support Coordinators, families, and the interdisciplinary team.
• Provide caregiver training and home programs.
• Adhere to DDD policies and HIPAA confidentiality.
• Maintain accurate billing records per DDD rates.
