Job summary

Senior Care Partners PACE seeks a compassionate Home Health Aide to assist participants with daily living activities in home, community settings, and day centers. You will support shopping, laundry, meal prep, and medication reminders under supervision, helping participants maintain independence and dignity.

The role requires at least a high school diploma, reliable communication, and one year of experience with the frail elderly.

Qualifications

• Must have a high school diploma or equivalent.
• Good written and verbal communication skills.
• Detail oriented with good problem-solving abilities.
• Basic computer skills.
• Minimum of one year working with frail, elderly, or long-term care population.

Responsibilities

• Provides ADLs (shopping, laundry, meal preparation, housekeeping, etc.) in the participant's home.
• Observe, record, and report changes in the participant’s condition to the care team.
• Support and respect the participant’s autonomy and dignity through respectful communication.
• Encourage maximum independence during care.
• Collaborate with staff and the participant’s family using a participant-centered care approach.
