Job summary

PACE by IIH is seeking a compassionate Certified Nursing Assistant to support seniors at the center, assisting with activities of daily living, personal care, and mobility to help participants remain independent.

Under the supervision of the Center Manager, you will document care, report changes in condition, participate in care planning, escort participants to appointments, and collaborate with an interdisciplinary team to ensure safety, dignity, and high-quality home-based care.

Qualifications

• Minimum one year of experience with frail or elderly population.
• Experience in long-term care facility or community geriatric program preferred.
• Must possess active California Certified Nursing Assistant license.
• CPR certification with First Aid upon hire; food handler training within 60 days if required.

Responsibilities

• Assist with activities of daily living (toileting, showers, eating, grooming).
• Provide nursing care as assigned per center policies.
• Maintain participant safety and report safety hazards/changes in condition.
• Escort participants to medical and other appointments as needed.
• Maintain written records and participate in care planning and meetings.
