Job summary

Habitat Health is seeking a compassionate Nursing Assistant to support daily living and safety for participants in our PACE center and in-home visits. You will assist with feeding, mobility, hygiene, and routine tasks while following care plans.

Responsibilities include documenting in the EMR, coordinating with care teams, and aiding participants with appointments. The role requires a CNA course completion, CPR/BLS, and a genuine dedication to elder care.

Qualifications

• Completion of accredited vocational nursing assistant course or comparable participant care experience.
• Food Handler's Certification required; must obtain within 30 days of start date.
• Ability to work without close supervision and exercise independent judgment.
• Knowledge of care needs of frail older adults.
• One year experience working with the frail or elderly.
• Proof of immunizations, TB test, CPR/BLS certification.

Responsibilities

• Attend to daily living and safety needs of participants in PACE center and home.
• Assist participants with activities of daily living (ADL).
• Prepare and plate meals per dietary cards and special needs.
• Record participant care data in the EMR accurately and timely.
• Accompany participants to appointments and conduct reminder calls.
• Maintain cleanliness and organize participant care areas.
