Job summary

St. Paul’s PACE Corp is seeking a Home Health Aide to provide compassionate in-home care to PACE participants under RN supervision, aligned with the IDT plan and California regulations.

Responsibilities include personal care, mobility assistance, meal support, and safety monitoring in participants’ homes within the PACE program. CA HHA certification and at least one year of elder care experience are required.

Qualifications

• Education: Must have completed CDPH-approved pathway to HHA certification and have an active California Home Health Aide (HHA) certification.
• Experience: A minimum of one year’s experience working with frail elderly population.

Responsibilities

• Personal hygiene and bathing assistance.
• Mobility, transfers, and positioning support.
• Assistance with meals and therapeutic diets.
• Light housekeeping and related care tasks.
• Medication assistance: assist only with self-administered meds; do not administer.
• Report changes to RN/therapist and document care in a timely manner.
• Deliver services per IDT plan and PACE policies.
• Follow infection control and safety procedures.
• Observe and report vital signs if taught and delegated.
