Job summary

NeighborHealth is seeking a Part-time Per Diem Nursing Assistant/Home Health Aide to cover day room staff absences at the PACE Center and to escort participants to external appointments. The role includes helping with personal care, meals, and activities, and may involve assisting with transportation safety and support across sites.

Ideal candidates will have a High School diploma or GED, and a Nursing Assistant or Home Health Aide certification, with experience with older adults.

Qualifications

• High School diploma or GED required.
• Certification as Nursing Assistant or Home Health Aide required.
• Experience with older adults or frail populations preferred.

Responsibilities

• Assist Center participants with personal care, meals, and activities.
• Escort participants to appointments and accompany transportation as needed.
• Support care planning and report to the Activities Coordinator, Nurses and Rehabilitation staff.
• May float to other sites to support patient care needs across the program.
