Job summary

NeighborHealth seeks a Part-time Home Health Aide for the PACE Community Care program in the Boston area. You will provide in-home ADL/IADL assistance to participants, work independently with minimal supervision, and support care plans in a respectful home setting.

Schedule: Monday–Friday, 7a–11a, with every other weekend/holiday rotation. Requires HHA/CNA, 1+ year frail/elderly care, reliable transportation, and strong customer service; bilingual a plus.

Qualifications

• High School graduate or equivalent.
• HHA/CNA certification from an approved program.
• Minimum of one year experience with frail or elderly population.
• Vehicle to travel from home to home required.
• Excellent customer service and communication skills; bilingual a plus.
• Physical capability to provide ADL and mobility assistance.

Responsibilities

• Monitors ADL/IADL care plans and documents care on flow sheets.
• Checks voicemail at shift start and every two hours, reporting abnormalities.
• Assists participants with ADLs per care plan and as needed (e.g., toileting).
• Accompanies participants to appointments as assigned.
• Carries out simple treatments under nursing supervision (e.g., catheter care, skin checks).
