Qualifications

• Registered Nurse fully licensed by Mass Board of Registration.
• Five years of home health care experience.

Responsibilities

• Conduct comprehensive in-home assessments for new AFC applicants.
• Evaluate medical, functional, and psychosocial conditions.
• Partner with caregivers to design customized care plans.
• Provide education to participants regarding AFC services.
• Ensure compliance with state regulations and AFC requirements.
• Maintain documentation of assessments and care plans.
