The role emphasizes coordination with the Nursing department, to support quality care, compliance, and collaboration with interdisciplinary teams to maintain accurate assessments and care plans.

Qualifications

• Must be graduate from an accredited nursing program or hold a nursing degree.
• Must have current RN license in the state.
• Experience coordinating MDS and care plans with regulatory timelines.
• Strong understanding of facility regulations and quality standards.

Responsibilities

• Coordinate development and completion of the resident assessment (MDS) per RAI Manual.
• Manage RAI process and care plans per VHS standards.
• Develop and maintain an ongoing quality assurance program for the assessments.
• Schedule resident assessment/care plan meetings and inform supervisors accordingly.
• Ensure MDS transmission within regulatory time frames.
• Sign and date MDS and ensure documentation accuracy.
• Assist in contacting care plan team members and resident representatives.
