Kaizen Lab Inc. is seeking an MDS Nurse Assessment Coordinator to join the Brockton Home Health AFC team. This position involves conducting in-home assessments for new applicants within the Adult Foster Care program and ensures eligibility based on their unique needs.

The ideal candidate will have a strong nursing background, ideally with prior home health care experience, and must possess an RN license from the Massachusetts Board of Registration. Benefits include comprehensive health coverage and a supportive work environment.

Qualifications

• Demonstrated ability to coach, train and provide staff development.
• Must be fully licensed RN by Mass Board of Registration.
• At least five years of previous home health care experience.

Responsibilities

• Conduct comprehensive in-home assessments for new AFC applicants.
• Evaluate medical, functional, and psychosocial conditions.
• Partner with caregivers and healthcare professionals to design customized care plans.
