Brockton Home Health AFC is seeking a detail-oriented Registered Nurse (RN) as an MDS Nurse Assessment Coordinator. This role focuses on conducting in-home assessments for the Adult Foster Care program, ensuring eligibility, and creating personalized care plans with a dedicated team. Qualifications
- Must be fully licensed RN by the Mass Board of Registration.
- Five years of previous home health care experience.
Responsibilities
- Conduct comprehensive in-home assessments for new AFC applicants.
- Evaluate medical, functional, and psychosocial conditions.
- Design customized care plans with caregivers and families.
- Educate participants and families about AFC services.
- Ensure compliance with state regulations and AFC requirements.
- Document assessments, care plans, and follow-up activities.
