Brockton Home Health AFC is seeking a Registered Nurse (RN)/Case Manager for the MDS Nurse Assessment Coordinator position in Brockton, MA. This role requires conducting in-home assessments for the Adult Foster Care program to enhance the quality of life for participants.

Qualifications

• At least five years of previous home health care experience.
• Experience in geriatrics or community-based care is preferred.
• Ability to conduct in-home evaluations.

Responsibilities

• Conduct comprehensive in-home assessments for new AFC applicants.
• Evaluate medical, functional, and psychosocial conditions to determine eligibility.
• Design customized care plans in collaboration with caregivers and families.
• Educate participants and families about AFC services.
