Job summary

21st Century Home Health Services is seeking a Full Time Physical Therapist Case Manager, Level 2, to serve the Cloverdale, CA area. You will evaluate patients in homes, address needs, and help clients live safely at home while restoring function and maximizing independence.

In partnership with clinicians, you will supervise 2 PTAs, develop care plans, and coordinate with physicians to ensure high-quality care. Prior home health case management experience is valued.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Knowledge of state/federal homecare guidelines preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case management fundamentals are highly desired.

Responsibilities

• Perform an in-home evaluation and develop a care plan with PTAs following the plan.
• Help patients regain independence through mobility and therapeutic activities.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients' physicians to maintain quality of care.
• Evaluate and report patient progress to the physician.
• Supervise 2 Physical Therapy Assistants to ensure performance standards.
