Job summary
21st Century Home Health Services Inc. is seeking a Full Time Physical Therapist Case Manager, Level 2, for the Cloverdale, CA area. You will collaborate with clinicians to provide skilled services to help individuals live safely in their homes.
Your primary responsibilities include evaluating and treating patients, supervising PTAs, and developing care plans. The position offers flexible work schedules, autonomy, and comprehensive benefits including competitive compensation and an achievable incentive bonus program.
Qualifications
- Must have a valid CA Driver's License.
- 1+ year experience supervising PTAs in a home health environment is a plus.
- Experience in home health care is highly preferred.
Responsibilities
- Perform an evaluation in the home and develop a care plan.
- Help patients with mobility and regaining their independence.
- Create detailed treatment and care plans for each patient.
