Job summary

21st Century Home Health Services is seeking a Physical Therapist Case Manager, Level 2, to serve the Healdsburg, CA area. You will assess patients in their homes, design care plans, and supervise PTAs to deliver effective treatment in a home health setting.

Ideal candidates hold a CA PT license, BLS/CPR, and have experience with interdisciplinary teams and high-acuity patients. The role offers flexible scheduling and strong benefits as part of a patient-centered, growth-focused organization.

Qualifications

• CA Physical Therapist License in good standing.
• 1+ year supervising PTAs in HH is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience with high acuity patients preferred.
• Experience with interdisciplinary teams preferred.

Responsibilities

• Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
• Help patients with mobility and regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
