21st Century Home Health Services Inc. is seeking a Physical Therapist Case Manager, Level 2 for the Concord, CA area. You will evaluate patients in their homes, develop and implement care plans, and supervise PTAs to restore function and independence.

The role emphasizes collaboration with physicians and a patient-centered approach. Ideal candidates hold a CA PT license, have BLS/CPR certification, and bring experience with home health care.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local home health care regulations preferred.
• Experience with high acuity patients is a plus.

Responsibilities

• Perform in-home evaluations and develop care plans with PTAs.
• Supervise PTAs to ensure adherence to plan of care.
• Develop detailed treatment and discharge plans and progress notes.
• Coordinate care with patients' physicians to ensure quality.
• Report patient progress to MD and adjust care as needed.
