Job summary

21st Century Home Health Services (21HHS) is seeking a Physical Therapist Case Manager to deliver in-home PT services across Santa Cruz County. You will assess, plan, and implement treatment, collaborating with PTAs and physicians to support safe, functional independence for patients.

The role requires a CA PT license, CPR/BLS, and a valid CA driver’s license. Excellent benefits, mileage reimbursement, and growth opportunities are offered.

Qualifications

Current unrestricted CA Physical Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources is required.
Knowledge of home health care regulatory guidelines.
Experience with high acuity patients is a plus.
Experience working with an interdisciplinary team is highly desired.

Responsibilities

Evaluate and address patients in their homes to restore function.
Evaluate and treat patients using prescribed physical therapy programs.
Supervise PTAs per agency and regulatory standards.
Communicate with patients’ physicians to ensure quality of care.
Develop and follow care plans; report patient progress to MD.
