Job summary

21st Century Home Health Services in Gilroy, CA is seeking a Full Time Physical Therapist Case Manager, Level 1 to provide skilled services in patients' homes across Santa Clara County.

You will evaluate and treat patients, develop care plans, supervise PTAs, coordinate with physicians, and help patients regain independence. A CA PT license, BLS/CPR, and a valid CA driver's license are required; knowledge of local resources and home health regulations is preferred.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Knowledge of home health regulatory guidelines preferred.
• Experience home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Self-Directed; Empathetic; Critical thinker; Leadership; Organized.

Responsibilities

• Evaluate and address the needs of individuals in their homes to enable safe, productive living.
• Restore function and maximize client performance within their capabilities.
• Evaluate and treat patients using medically prescribed physical therapy programs.
• Supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards.
• Develop care plans and coordinate with patients’ physicians to ensure quality care.
