Job summary
21st Century Home Health Services seeks a Full Time Physical Therapist Case Manager in Santa Rosa, CA. This role involves evaluating patients in their homes, creating treatment plans, and working with PTAs. Qualifications
- Ability to communicate and coordinate with patients’ physicians to ensure a quality level of care.
- Experience with high acuity patients is a plus.
- Self-directed and organized approach to work.
Responsibilities
- Perform evaluations in the home and develop care plans.
- Help patients with mobility and regain independence.
- Evaluate and report patients’ progress to MD.
Skills
Current unrestricted CA Physical Therapist License
Knowledge of local community resources
Outstanding interpersonal skills
Experience in home health care
Critical thinking
Education
Current BLS/CPR Certification
Valid CA Driver’s License
