21st Century Home Health Services (21HHS) is seeking a Part-Time Physical Therapist Case Manager to deliver in-home evaluations and therapy across the Vallejo area. You will develop and adjust care plans, coordinate with PTAs, and support patients toward functional independence.

Ideal candidates hold an unrestricted CA PT license, current BLS/CPR, and a CA driver’s license. The role emphasizes patient-centered care, teamwork, and flexible scheduling in a field-based setting.

Qualifikationen

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources and cultural considerations in care.
• Experience in home health care is preferred.

• Perform home visits, evaluate needs, and develop care plans.
• Coordinate with PTAs to implement plans and monitor progress.
• Help patients regain mobility and independence.
• Communicate with patients’ physicians to ensure quality care.
