Job summary

21st Century Home Health Services (21HHS) is seeking a dedicated Physical Therapist Case Manager to join our field-based team delivering in-home care. You will evaluate patients, develop care plans, and coordinate with physicians to ensure high-quality outcomes.

Requirements include a CA Physical Therapist license, BLS/CPR, and experience supervising PTAs in home health. You’ll work in a collaborative interdisciplinary environment with autonomy and flexible scheduling.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a home health environment.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of state and federal homecare regulatory guidelines.
• Experience in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals are highly desired.

Responsibilities

• Perform an evaluation in the home and develop a care plan; collaborate with PTAs to follow the plan.
• Help patients with mobility and regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
