Job summary

21st Century Home Health Services is seeking a Full Time Physical Therapist Case Manager to provide in-home skilled PT services across Monterey, CA. You will evaluate patients, develop and implement care plans, and collaborate with other clinicians to maximize function and safety in the home setting.

Responsibilities include evaluating home environments, coordinating with physicians, and reporting progress.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Experience in home health care is highly preferred.

Responsibilities

• Evaluate the patient in the home and develop a care plan with PTA supervision.
• Help patients regain mobility and independence.
• Create detailed treatment and care plans.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate patient progress and report to the MD.
