Job summary
A leading home health agency is seeking a Full Time Physical Therapist Case Manager to provide skilled services in the Folsom area. Qualifications
- Knowledge of local community resources is required.
- Knowledge of state and federal homecare regulatory guidelines preferred.
- Experience with high acuity patients is a plus.
Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs.
- Help patients with mobility and regaining independence.
- Communicate and coordinate with patients’ physicians.

Current unrestricted CA Physical Therapist License
Current BLS/CPR Certification
