Job summary

21st Century Home Health Services Inc. seeks a Full Time Physical Therapist Case Manager, Level 2, based in Berkeley, CA. The successful candidate will need a valid CA Physical Therapist License and strong interpersonal skills. Qualifications

• 1+ year experience supervising PTAs in a home health environment is a plus.
• Knowledge of local community resources is required.
• Experience in home health care highly preferred.

Responsibilities

• Perform an evaluation in the home and develop a care plan while working with PTAs.
• Help patients with mobility, movement, and regaining independence.
• Communicate and coordinate with patients’ physicians to ensure quality care.
