Job summary
A leading home health agency is seeking a Part Time Physical Therapist Case Manager to work in the Sacramento area. This role involves evaluating patients in their homes and creating personalized care plans to help them regain independence. The ideal candidate will hold a current CA Physical Therapist license, have strong interpersonal skills, and experience in home health care. This position offers competitive compensation with a base salary of $120,000 - $135,000 plus annual bonuses and comprehensive benefits.
Qualifications
- Experience working with high acuity patients is a plus.
- Knowledge and understanding of individual development and human behavior as it relates to illness/injury.
- Experience working with an interdisciplinary team is highly desired.
Responsibilities
- Perform an evaluation in the home and develop a care plan.
- Help patients with mobility and regaining independence.
- Communicate and coordinate with patients’ physicians.
Skills
Current unrestricted CA Physical Therapist License
Current BLS/CPR Certification
Valid CA Driver's License
Knowledge of local community resources
Experience in home health care
Critical thinker
Outstanding interpersonal skills
