Job summary

A leading home health agency in California seeks a Part Time/Weekend Physical Therapist Case Manager to provide skilled services in patients' homes. The ideal candidate will facilitate patient rehabilitation, conduct evaluations, and develop care plans. Flexibility with work schedules, competitive compensation between $120,000 to $135,000 annually, plus benefits such as medical, dental, and a 401k plan are offered. A CA Physical Therapist License and experience in home health care are required.

Qualifications

• Experience in home health care is highly preferred.
• Knowledge of local community resources is required.
• Knowledge of state and federal homecare regulatory guidelines preferred.

Responsibilities
