Qualifications

• Experience in home health care is highly preferred.
• Experience working with an interdisciplinary team is desired.
• Knowledge of local community resources is required.

Responsibilities

• Perform an evaluation in the home and develop a care plan.
• Help patients with mobility and regain independence.
• Communicate and coordinate with patients’ physicians.

Skills

Current unrestricted CA Physical Therapist License

Current BLS/CPR Certification

Knowledge of human behavior related to health care

Leadership skills

Outstanding interpersonal skills

Self-Directed

Knowledge of state and federal homecare regulatory guidelines
