Job summary

21st Century Home Health Services Inc. is seeking a Full-Time Home Health Physical Therapist Case Manager, Level 2 in the Martinez, CA area.

You will evaluate patients in their homes, develop and implement treatment plans, and collaborate with a team of clinicians to restore function and independence. You will supervise PTAs, coordinate with physicians, and monitor progress to achieve optimal outcomes in a compassionate, patient-centered setting.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a Home Health environment.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Understanding of culture's influence on health care.

Responsibilities

• Perform home evaluations and develop care plans in collaboration with PTAs.
• Address mobility and independence through skilled PT interventions.
• Supervise PTAs to ensure adherence to care plans and performance standards.
• Communicate and coordinate with patients' physicians to maintain high-quality care.
• Evaluate patient progress and report to the attending MD.
