Job summary
21st Century Home Health Services in Grass Valley seeks a Full Time Physical Therapist Case Manager to evaluate and address home health needs, helping patients live safely and productively at home. You will develop individual care plans, rehabilitate mobility, and coordinate with physicians and PTAs to ensure high-quality outcomes.
Requirements include a CA PT license, BLS/CPR, a valid CA driver's license, and knowledge of local community resources.
Qualifications
- Current unrestricted CA Physical Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver's License.
- Knowledge of local community resources.
- Experience in home health care is highly preferred.
Responsibilities
- Evaluate patients in home settings and develop care plans with PTAs.
- Help patients regain mobility and independence.
- Communicate with physicians to ensure quality care.
- Report patient progress to the MD and supervise PTAs as needed.
- Coordinate interdisciplinary care and documentation.
