Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations. The role involves visiting patients in their homes, reviewing medical history, performing physical assessments, and coordinating with health plans to bridge gaps in care.

Clinicians must hold an active MD/DO license with multi-state licensure preferred, and be willing to travel locally to patient homes and within licensed states. Virtual visits may be conducted as needed.

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Visit patients in home setting and perform health evaluations.
• Collect medical history and conduct physical assessments.
• Coordinate care with health plans and care teams.
• Travel locally as needed to patient homes and licensed states.
• Conduct virtual visits as needed and document findings.
