Job summary

Signify Health, part of CVS Health®, seeks a Full Time Physician to provide In-Home Health Evaluations, engaging with patients in their homes and bridging gaps in care.

You’ll perform comprehensive health assessments, document medical history, and coordinate with health plans to connect patients to the right care. Travel locally within licensed states is required; virtual visits may be used as needed.

Qualifications

• Active, unrestricted license in the coverage area.
• MD or DO degree required; board certification preferred.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Engage with patients during home visits and document findings.
• Coordinate care and referrals with health plans and PCPs.
• Operate with a clinician mindset to improve health outcomes.
• Travel locally to member homes and conduct virtual visits as needed.
