Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit, which lasts about 45 minutes, includes a medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered by the health plan. You may travel locally and within licensed states, with virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in the coverage area.
• MD or DO degree required; board certification preferred.
• Willingness to travel locally and within licensed states.

Responsibilities

• Provide in-home health evaluations (~45 minutes) for health plan members.
• Review medication history and perform physical examinations.
• Coordinate diagnostics and referrals as ordered by the health plan.
• Travel locally and within licensed states; conduct virtual visits as needed.
