Job summary

CVS Health Corporation is seeking a full-time physician to perform in-home health evaluations for Signify Health, providing personalized assessments at patients’ homes. The visit includes history review, physical exam, and coordination with health plans to bridge gaps in care.

Reqs include active, unrestricted MD/DO licenses and multi-state licensure preferred. Travel locally and across licensed states is expected; virtual visits may be conducted as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to members in their residences.
• Perform medication and medical history review, physical evaluation, and discuss care plans.
• Utilize iPad and clinical workflow; no prescribing, ordering labs, or altering treatment unless specified.
• Travel locally and within licensed states; conduct virtual visits when needed.
