Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, visiting patients in the comfort of their own homes.

You will conduct assessments lasting about 45 minutes, review medications and medical history, perform physical evaluations, and coordinate with health plans. Local travel within licensed states is required, with some virtual visits as needed.

Qualifications

• Active, unrestricted medical license(s) in applicable states.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for patients.
• Perform medication and medical history reviews and physical evaluations.
• Coordinate with health plans and care teams.
• Travel locally within licensed states; may conduct virtual visits.
