Job summary

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations, engaging with people in the comfort of their own homes and bridging gaps in care.

You will connect patients with the right care, conduct physical evaluations, and coordinate with health plans. This role requires an active MD/DO license and willingness to travel, including local travel and visits to licensed states. Virtual visits are possible.

Qualifications

• Active/unrestricted license(s) in coverage area(s) is required.
• Multi-state licenses are preferred.

• Be flexible to travel locally and within licensed states, as needed.

• Provide all Signify Health services including Diagnostic & Preventive Services such as spirometry and other screenings.
