Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to perform in-home health evaluations for plan members. The role involves visits at patients' homes, reviewing medical history, conducting physical assessments, and coordinating with care teams.

Travel to licensed states is expected, and virtual visits are also possible. Eligible candidates are MDs or DOs with active licenses; multi-state licenses preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations during 45-minute visits.
• Visit patients nationwide to connect them with the right care.
• Collaborate with Signify Health as a clinician colleague.
• Travel locally within licensed states as needed.
• Conduct virtual visits as needed.
• Deliver Diagnostic & Preventive Services such as spirometry and bone density screening.
