Job summary

Signify Health, part of CVS Health, is hiring a Full-time Physician to provide in-home health evaluations, engaging with members in the comfort of their homes and bridging gaps in care. You’ll build trusted relationships and deliver personalized assessments.

The role involves 40 hours per week, travel to local and licensed-state locations, and use of iPad-based workflows. Prior MD/DO licensure and multi-state credentials are preferred.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Multi-state licenses (preferred).
• Able to cover primary locations: Alpena and surrounding counties.

Responsibilities

• Provide in-home health evaluations for members.
• Review medications and medical history during visits.
• Perform physical evaluations and document findings.
• Attend to diagnostic tests as ordered by the health plan.
• Travel locally to member homes and licensed states as needed.
• Conduct virtual visits when required.
