Job summary

Powerback Rehabilitation seeks a licensed Physical Therapist Assistant to provide outpatient rehabilitation services in home-based settings for older adults. You will evaluate and treat in patients’ homes or living facilities, under supervision of a Licensed Physical Therapist, with a flexible schedule across various settings.

The role emphasizes patient-centered care, functional training, and collaboration with a rehab team to enhance independence and quality of life for clients.

Qualifications

• Must hold a valid license in the state(s) of practice.
• Must have a valid driver’s license or reliable transportation to and from patients’ homes.

Responsibilities

• Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy, under the direction of the physical therapist.
• Ensures adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.
• Collaborates with rehabilitation team to implement programs and activities suited to each patient’s needs and capabilities.
• Assists the Physical Therapist when tests, evaluations, and complex procedures are performed.
• Instructs patients, families and caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the Physical Therapist.
• Maintains appropriate and timely documentation for all patients treated.
• Cleans, maintains, and reports equipment repairs as needed.
• Delivers patient-centered care and implements individualized treatment plans.
• Provides a range of PT treatments including therapeutic exercise, gait training, manual therapy, airway techniques, and functional training.
