Job summary

UnitedHealth Group, through Deaconess HomeCare, seeks a Physical Therapist Assistant to deliver treatment and education per the plan of care. You will guide patients through exercises, monitor progress, and collaborate with the care team to improve well-being.

This role requires state PT assistance licensure, a valid driver’s license, and weekend availability. A CPR certificate is preferred or can be completed within 90 days of hire.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance or access to a dependable vehicle or public transportation.
• Available to work a weekend rotation.
• CPR certification preferred or able to obtain within 90 days of hire.
• Ability to perform physical tasks required for patient care (lifting, transferring, mobility).

Responsibilities

• Follow state-specific laws governing physical therapy in home care and execute plan of care under supervising PT.
• Instructs and assists patients with active and passive exercise, gait, ADL, transfer, and safety.
• Observe and report patient responses; coordinate care with health care team.
• Educate patient, family, caregiver, and team on therapy within scope of PTA.
• Function effectively in diverse home environments.
