Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residence.

Responsibilities

• Attends and contributes to patient care, staffing conferences and meetings as a representative of physical therapy.
• Adheres to department and patient treatment schedules and documentation requirements.
• Provides outpatient rehabilitation services in in-home and community settings under supervision of a licensed PT.
• Instructs patients and families in skills of the physical therapy program.
• Maintains documentation and reports equipment needs.
• Implements patient treatment plans and conducts therapeutic exercises, functional training, manual therapy, and modalities.
