Powerback Rehabilitation is seeking a Physical Therapist to provide in-home outpatient rehabilitation for older adults, evaluating and treating patients in their homes or independent living facilities.

You will develop individualized therapy programs, coordinate with the rehabilitation team, supervise assistants as permitted, and maintain thorough documentation to ensure high-quality care. Posted salary: USD 79,000–110,000 per year.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy (or PT-related master’s with a PT bachelor’s).
• Bachelor’s degree in Physical Therapy with 5+ years of progressive PT experience.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults.
• Develop, implement, and adjust individualized PT treatment plans.
• Refer to other disciplines as needed to address comprehensive patient needs.
• Educate patients and caregivers to promote independence and safe home mobility.
• Supervise PT assistants and students as allowed by licensure.
• Maintain timely clinical documentation and participate in care conferences.
