Job summary

Powerback Rehabilitation To You in Bedford, NH is seeking a Physical Therapist Assistant for mobile outpatient therapy, delivering rehabilitation in-home or in independent living settings. The role emphasizes patient-centered care and flexible scheduling to meet older adults where they live and thrive.

You will work under a licensed PT, perform a range of therapeutic interventions, document care, and contribute to a supportive, fast-paced team that values mentorship and growth.

Qualifications

• Must have a valid license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.

Responsibilities

• Attends and contributes to patient care and required meetings as a representative of physical therapy, under the direction of the PT.
• Ensures adherence to department and patient treatment schedule, clinical and billing documentation.
• Collaborates with rehab team to implement programs for each patient.
• Assists PT during tests, evaluations, and complex procedures.
• Instructs patients and caregivers in skills of the PT program under PT supervision.
• Maintains timely documentation for all patients treated.
• Cleans, maintains, and reports needs for equipment repair.
• Delivers patient-centered care aligned with individualized treatment plans.
