Qualifications

• Graduate from an accredited physical therapy program (CAPTE)
• Current license to practice physical therapy (state-specific)
• Driver’s License and access to reliable transportation
• Current CPR certification
• 1+ years related physical therapy experience in home health care
• If <1 year, regional clinical leaders’ approval and mentoring/residency participation

Responsibilities

• Help patients maintain, restore and improve movement, activity and function
• Provide interventions, conduct reexaminations at least every 30 days, modify interventions as needed
• Perform discharge planning
• Other duties as assigned
