Job summary

Amedisys, part of the Optum family, is hiring a Physical Therapist - HAC to restore function and mobility for patients in a home health setting. You will provide skilled PT services under physician-directed plans of care within a interdisciplinary team, focusing on assessment, treatment planning, and coordination of care.

Responsibilities include patient assessments, developing plans of care, ongoing evaluations, and collaboration with physicians.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of a CAPTE-accredited physical therapy program.
• 1+ year of experience in physical therapy; OR <1 year with an approved residency/mentorship.
• Current CPR certification.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Driver's license and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized PT plans of care with physicians.
• Provide skilled PT interventions and modify plans based on progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and changes in condition.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
